Meals on Wheels San Francisco works to help seniors live independently, in their own homes, for as long as it is safely possible.  The Social Work department plays an integral role in helping seniors stay healthy and safe.   

 The Social Worker is responsible for conducting initial and annual in-home assessments with homebound senior clients, making appropriate referrals for additional community services, keeping updated with resources for seniors, working closely with the home delivered meal staff to ensure smooth delivery of meal service to clients, handling client situations as needed, and providing case management for a small caseload of clients.

Under the supervision of the Social Work Supervisor, and in accordance with the provisions of MOWSF’s

Employee Handbook and the Social Work and Case Management Procedures Manuals, the Social Worker is

responsible for:

  1. Conducting 15 in-home initial assessments weekly as well as conducting  annual assessments throughout all areas of San Francisco with clients on the home delivered meal program for determining eligibility, appropriate meal type and other social service needs.

     2. Maintaining a confidential file on each client served in the fiscal year in a legible and professional manner including accurate and timely documentation.

    3.    Establishing and maintaining appropriate records, preparing reports and other correspondence and entering client data into MOWSF and DAAS databases as required.

  1. Making appropriate referrals to community resources, and coordinating services for effective client care.
  1. Assisting with development and updating of information on community resources for seniors.

      6.  Keeping updated with and carrying out all MOWSF agency, social work and client policies.

  1. Responding to client inquiries about the program, handling client situations as they arise, and conducting new client intakes as needed.
  1. Working with the Home Delivered Meal staff, driving staff, and other MOWSF departments to assure clients are effectively served.                                                   
  1. Meeting with drivers weekly to monitor clients’ eligibility and safety, complete quarterly reassessments and address any other concerns or needs.
  1. Following up with clients not answering the door for their meal delivery.
  1. Providing limited case management to seniors in need of critical support: preparing care plans, providing ongoing case monitoring and coordination of services, and assessing for additional needs.
  1. Referring clients to MOWSF volunteer services, handling client/volunteer concerns as needed.
  1. Assisting with the Volunteer Orientations for new MOWSF client volunteers as needed
  1. Attending relevant community senior and social service meetings as assigned.
  1. Participating in presentations of social work information at driver meetings, in-service workshops, outside agency presentations, etc. as requested.
  1. Providing computer and data entry assistance in the Social Work Department as needed
  1. Working at MOWSF events as requested such as Thanksgiving Day.
  1. Providing monthly reports to the Social Work Supervisor.
  1. Delivering meals in emergency situations as needed.
  1. Performing other duties as assigned




  • A Bachelor’s or Master’s degree in Social Work or Gerontology is required.
  • May consider a degree in a related social service field along with relevant work experience
  • Preference given for two or more years of work experience with seniors and familiarity with resources for seniors in San Francisco  
  • Ability to work with a diverse community.
  • Must be able to work well with others as part of a team  
  • Excellent organization, communication & written documentation skills
  • Computer experience, including Microsoft Office, some database familiarity and proficiency in typing 
  • Bilingual/bicultural a plus, most needed languages: Spanish, Cantonese, Russian, Mandarin,Tagalog
  • Valid driver’s license, proof of insurance and automobile required
  • A background check including a DMV printout is part of the application process.
